Sounds almost like he's having some nice seizures, with grimacing and loss of muscle control. The confusion may be the recovery stage. On the other hand, the episode when he wrote himself a note is inconsistent as he wouldn't have the control to write if he had a seizure bad enough to lose memory. Any substance use? Regardless, impossible to tell without seeing the doctor, though. It would certainly be very exciting if he had one when he was driving - that would be definitely be one way he'd end up seeing a doctor.
